New patients will need to fill out forms prior to our first appointment.
I am contracted with Blue Cross/Blue Shield, Cigna, Tufts, Allways (Neighborhood Health Plan), and Harvard Pilgrim, and some versions of Cigna and United. Each plan provides a different benefit for nutrition counseling. Some treat it the same as an office visit to your doctor, others limit visits to a certain number per year, others are based on medical necessity to be determined by myself and your primary care physician. Prior to our first appointment, I will have you fill out a form with your insurance information and my billing office will verify the benefit (minimize surprises!)
*I do not accept MassHealth or Medicare due to limitations within those plans. Secondary insurance plans will not cover nutrition services if MassHealth or Medicare are primary. It's extremely confusing. I know. If you believe that your MassHealth plan will negotiate a single case agreement, we will need to set that up in advance of an appointment.
